Firm-X™ Penile Botox for Enhanced Erections

Firm-X™ is what's often called penis botox: botulinum toxin (Botox, Xeomin or Dysport) injected into the corpus cavernosum, the erectile tissue of the penis. It relaxes the smooth muscle there, so blood flows in more easily when you're aroused, and erections are firmer.

The evidence is strong. In two randomized, placebo-controlled trials, a single injection improved erections, and in the larger one the effect lasted up to six months.^{1, 2} The details are below.

Botulinum toxin is FDA-approved for other uses; using it for erectile dysfunction is off-label.

Abdelrahman et al., 2022. 70 men whose ED hadn't responded to pills were given one injection of botulinum toxin or a salt-water placebo, and kept taking their pills. At six weeks, 65 percent of the treated men reported better erections and 53 percent could have an erection firm enough for penetration, against one man on placebo, with no complications reported.¹

El-Shaer et al., 2021. A randomized, placebo-controlled trial of 176 men with erectile dysfunction caused by blood flow. The injection improved erection hardness and blood flow in the penis, and the effect lasted up to six months.²

Giuliano et al., 2022. 123 men for whom pills or injections weren't doing enough had botulinum toxin added to their treatment. For 41 percent of them the benefit lasted at least six months, and the only side effects were two cases of brief discomfort.³

Men with erectile dysfunction, including men whose pills haven't worked well enough, and men having filler who want firmer erections too. Men on blood thinners and men with severe ED can usually have it as well; the doctor confirms at your visit.

Every botulinum toxin product carries this warning from the FDA.⁶ The effect of the toxin can spread away from where it was injected. When that happens it can cause muscle weakness, blurred vision, drooping eyelids, trouble swallowing, trouble speaking, and trouble breathing. Swallowing and breathing problems can be life threatening, and deaths have been reported. Symptoms have shown up anywhere from hours to weeks after an injection. Get emergency help straight away if you have any trouble swallowing, speaking or breathing after treatment.

Tell your doctor before treatment if you have a nerve or muscle disorder such as myasthenia gravis, Lambert-Eaton syndrome or ALS, if you have had any swallowing or breathing problem, or if you have had any botulinum toxin injection in the last four months.
